You need 1 form per gun, each gun needs its own PTC. Have one Xerox copy for receiving copy for each application. The original will be submitted to the PNP. There're notaries in the PTC Office, and at the One-Stop Building. The notary at the One-Stop is cheaper and charges 50 per application. He didn't ask for a Xerox copy of the application.
