Differentiate between FDA-approved and non-FDA-approved uses of sodium bicarbonate while understanding the clinical evidence supporting each application.
Bicarbonate is an essential chemical regulating the acid-base balance acting as a buffer [2]. Carbon dioxide (CO 2) is a major byproduct of energy metabolism in living organisms and a conjugate acid. Carbonic anhydrase enzyme facilitates the chemical interaction between CO 2 and water producing carbonic acid (H 2 CO 3).
AKI is often associated with acidosis, and consequently, it has become common practice to recommend administration of alkaline, usually sodium bicarbonate, to correct any acidosis. We conducted this review to assess the evidence for the use of sodium bicarbonate for people with AKI.
Oral bicarbonate 1.2 mEq/kg in 3 divided doses, titrated to maintain serum bicarbonate in range 22–26 mmol/L: Placebo: 3 months: Not specified: De Brito-Ashurt et al., 2009 : UK: 134: 55: 4 or 5: 16 mmol/L < Bicarbonate < 19 mmol/L: Oral bicarbonate 600 mg 3×/day, increased as needed to maintain serum bicarbonate > 23 mmol/L: Usual care: 2 years
7 Δεκ 2021 · Our results indicated that oral sodium bicarbonate supplementation significantly increased serum bicarbonate levels, resulting in a slower decline in eGFR and a reduction in ACR, which is in agreement with an earlier study that also showed a slower decline in eGFR after bicarbonate supplementation.
