This document serves as the official standards, measures, and required documentation for PHAB national public health accreditation. It provides guidance on the meaning and purpose of a measure and the types and forms of documentation that are appropriate to demonstrate conformity with each measure.
The Public Health Accreditation Board (PHAB) implements and oversees the national accreditation process for state, local, tribal and territorial health departments. The PHAB Standards and Measures are a standard set of criteria used to assess public health capability at the state and local level.
This tool is a crosswalk between the Public Health Accreditation Board (PHAB) Accreditation Standards and Measures Version 1.5 and the Centers for Disease Control and Prevention (CDC) Medical Countermeasure Operational Readiness Review (MCM ORR) for Budget Period 4 .
In 2011, the Public Health Accreditation Board (PHAB) launched the first national, voluntary accreditation program for health departments and its standards require a CHA and CHIP and explicitly reference MAPP as a model to meet these standards.
