Report a traffic accident by filing an SR-1. You or your insurance agent, broker, or legal representative must complete an SR-1 report and send it to DMV within 10 days if someone is injured (no matter how minor the injury) or killed, or property damage is over $1000.
An electronic online crash report can now be submitted by going to www.state.nj.us/transportation/refdata/accident/selfreporting.shtm The electronic version will enable drivers to fill out a form online and receive an acknowledgment back within minutes. The online receipt will be proof of submission. Please make copy of the crash report. SECTION A
Report of Accidents. The driver of a vehicle involved in an accident resulting in injury to or death of any person, or damage to property of any one person in excess of five hundred dollars ($500) shall within ten days after such accident forward a written report of such.
California law requires trafic accidents on a California street/highway or private property to be reported to the Department of Motor Vehicles (DMV) within 10 days if there was an injury, death or property damage in excess of $1,000. Untimely reporting could result in DMV suspending a driver license.
The New Jersey Self-Reporting Crash form (SR-1) is used to report a crash that was not investigated by the police and it is the only form that can be used for this purpose. The electronic versions below enable NJ citizens to fill out a form online and receive acknowledgment back within minutes.
In order to eliminate the fraudulent use of crash reports, all state report forms cannot be printed or modified. Overlay code sheets can be printed. In cases where states have revised their reporting forms, both old and new formats are provided.
