This is an application for the waiver of reinstatement fees through the Bureau of Motor Vehicles (BMV). In order to have reinstatement fees waived, the applicant must meet the eligibility requirements listed below, which are outlined in R.C. 4510.10 and 4510.102.
If you cannot afford to pay driver’s license reinstatement fees, a BMV program may help you. The program is called the Driver’s License Reinstatement Fee Debt Reduction and Amnesty Program. If you qualify, the program lowers or completely waives fees to reinstate your driver’s license.
Apply for a Waiver. When a vehicle has failed at least one emission test and the owner has made efforts to have that vehicle repaired, a waiver may be issued at any of the 23 Full-Service E-Check Center. Visit the Ohio EPA website https://epa.ohio.gov for a list of locations.
If you had an Ohio driver license or permit and owe reinstatement fees on eligible offenses, you will automatically be enrolled in the program. You also may submit proof of indigence for the waiver, if applicable.
An Ohio E-Check Exemption/Extension Application must be completed and either taken to or mailed to the Ohio EPA E-Check field office, mailed to Ohio EPA E-Check, P.O. Box 1049, Columbus, Ohio 43216-1049, or taken to one of the 23 full-service testing stations.
To apply for a complete waiver of fees, a driver must complete BMV Form 2829 and submit proof of indigence. Drivers qualify as indigent by showing proof of participation in any of the following benefit programs:
