The Ohio Department of Medicaid (ODM) and the Ohio Department of Aging (ODA) administer four waiver programs for older adults and people who have disabilities who meet a nursing facility level of care.
Enrollment on the Ohio Home Care Waiver Program is available for Medicaid eligible individuals under the age of 60, who meet an Intermediate or Skilled Level of Care, and other program eligibility requirements.
The ODM 02399 form is used to request the waiver and can be obtained and submitted at the local County Department of Job and Family Services (CDJFS). Requests can also be made by calling Ohio Benefits Long Term Services and Supports (OBLTSS) at (844) 644‐6582. 4.
