Morbidity and mortality rises with increasing burned surface area and the patient’s age. Even small burns may be fatal in elderly people. Burns greater than 15% surface area (adult), greater than 10% (child) or any burn occurring in the extremes of age are considered serious.

• Partial thickness burns greater than 10% TBSA. • Burns that involve the face, hands, feet, genitalia, perineum or major joints. • Third degree burns in any age group. • Electrical burns, including lightning injury. • Chemical burns. • Inhalation injury. • Burn injury in a patient with preexisting medical disorders.
Burn size assessment. It is important to accurately assess the surface area involved and possible depth of the burn. The most experienced clinician, in burns, available should assess the patient. Surface area should be charted on an appropriate chart.
