The most commonly employed levels of amputation are the phalangeal and transmetatarsal, each of which can be single or multiple, and are sometimes performed in combination. Less common are amputations performed through the midfoot (Chopart and Lisfranc), and hindfoot (Syme) (Fig. 1).
A circumferential incision is ideally made at least 0.5 toe-diameter distal to the disarticulation site to ensure sufficient soft tissue for tension-free closure. Tension-free closure may be difficult to achieve with distal injuries because of the nail or the lack of viable soft tissue.

When the most distal skin bleeds within 3 minutes after release of the tourniquet, there is an 80% to 85% chance for successful healing of the amputation incision. If bleeding is prolonged beyond 3 minutes, the next higher level should be tested.
For a fifth metatarsal amputation, the shaft should be transected obliquely with an inferolateral-facing facet, leaving the uninvolved half to three-quarters of the shaft to preserve forefoot width and retain the insertion of the peroneus brevis.
