The Orphans Court provides forms for auditing and administration, guardianship, Abortion Control Act, Register of Wills, model forms of account and foreign adoption. Access Register of Wills and Orphans' Court forms.
For those judicial officers who prefer to file a paper form instead of the online form, click here to access a "pdf" form that can be filled out and printed but must then be signed and submitted to the AOPC's Judicial Services unit via traditional mail. Instructions are included.

PACFile is a service that provides the option to file documents with the courts electronically on both new and existing cases. At this time, filings can be submitted to the Supreme, Superior, Commonwealth, and certain Common Pleas courts.
