23 Οκτ 2018 · Next, you must fill out an application for a dog breeding license. Complete the required application and turn it in at the Animal Care Office in your local area responsible for your state. A complete license application includes: an APHIS Form 7003-A, a $10 application fee, and; a Taxpayer ID Form.
