The ISP identifies information about the individual and summarizes all assessment results. ODP utilizes a multifaceted assessment process to drive initial and ongoing ISP development in order to gain and capture person centered information to determine the individual’s needs and risk factors.
Supports Coordinators (SCs) Individual Support Plan (ISP) monitoring is designed to provide support to individuals and their families, allows for frequent communication to address current needs and to ensure individuals health and safety. In addition, monitoring allows for increased support to plan for services throughout the lifespan.
Printable Blank ISP Worksheet. This template is to be used by SC when developing the ISP. It can be printed and shared with team members to gather information for the ISP and is provided for the team to use during the ISP planning process.
The ISP is a comprehensive summary of planned services and supports for individuals receiving services through PA Office of Developmental Programs, including those in Allegheny County. An individual’s first ISP is developed as soon as a Supports Coordinator (SC) is provided.
