You must file an Application for Disability Retirement (PSRS-49) to be considered for the benefit. Benefits are subject to approval, denial, or renewal based on your medical documentation and your job description. PSERS reserves the right to review and/or revoke your disability benefit at any time.
Member Forms. To avoid delays or errors, before you complete an application for retirement or disability retirement form, PSERS strongly suggests you attend one of our retirement exit counseling sessions in your area presented by a PSERS retirement representatives.

If you are considering retirement due to health issues, discuss the SERS disability retirement benefit with your retirement counselor before you terminate employment. Contact your retirement counselor for retirement benefit estimates and counseling.
You, your employer, or someone acting on your behalf with your power of attorney, may file a Disability Benefit Application provided by SERS for disability benefits. Your personal physician also must provide a report stating the basis of your disability.
