Turns out I disabled all notifications for Podcast Addict. The player controls on the lock screen are treated by the OS as a notification. To fix it, I went to Apps > Search "Podcast Addict" > Notifications > toggle "Show notifications" on.
MIUI doesn't like it when apps try to customize the lock screen player. Turn off customization in Podcast Addict by going to Settings > Notifications > Player > uncheck custom layout
It's there if I drag the bar down, but not showing just on the bar when playing like all other players. Lock screen shows fine. All notifications are set to allow, nothing is silenced.

By default the app opens on the screen that shows the list of podcasts you subscribed to. Replace Podcast Addict default icons on your launcher with a widget called "Opening Screen". This 1x1 widget allows you to select the app default screen.
