10 Οκτ 2020 · Prerenal AKI is due to nonvolume responsive hepatorenal syndrome causing rapid renal failure in patients with acute or chronic renal failure. Most authors agree that bile casts require sustained liver disease and high levels of serum bilirubin.
In order to understand the pathophysiology of pre-renal AKI, it is first necessary to understand the processes that regulate blood flow to the kidney, maintain glomerular filtration pressure, and control the amount of fluid reabsorbed by the kidney.
15 Μαΐ 2020 · The pathophysiology of acute kidney injury (AKI) is multifactorial and complex. It has historically been classified as prerenal AKI caused by renal hypoperfusion and renal parenchymal AKI characterized by acute tubular necrosis (ATN) due to ischemia and nephrotoxicity.
Traditionally, AKI has been classified based on the anatomic location of the initial injury, as prerenal (functional response of structurally normal kidneys to hypoperfusion), intrinsic renal (involving structural damage to the renal parenchyma), or postrenal (congenital or acquired anatomic obstruction to the lower urinary tract).
In Pre-renal AKI, renal hypoperfusion leads to a decreased GFR (without damage to the renal parenchyma), as an adaptive response to various extra-renal insults. 62 It is known that maintaining a normal GFR is dependent on adequate renal perfusion.
