First dose: 6 mg rapid IV push; follow with NS flush. Second dose: 12 mg if required. Antiarrhythmic Infusions for Stable Wide-QRS Tachycardia Procainamide IV dose: 20-50 mg/min until arrhythmia suppressed, hypotension ensues, QRS duration increases >50%, or maximum dose 17 mg/kg given.
Procainamide is dosed in dogs at 2 mg/kg, slow IV bolus, to a maximum cumulative dose of 25 mg/kg over 10–15 minutes, continuing if needed as a constant-rate infusion (CRI) at 25–40 mcg/kg/min or at 10–20 mg/kg, every 6–8 hours, IM or SC.
Dog. Acute treatment: initial dose 2 mg/kg, IV or IM, or 0.66–1 mg/kg/h, CRI, for maximum 4 h. Longterm management: 1–4 mg/kg, PO, q 6–24 h. Cat.
