Procainamide Dosage in ACLS Procainamide should be used in patients with tachyarrhythmias who are deemed hemodynamically stable. Once the clinician has determined the cardiac rhythm by electrocardiogram, the clinician should begin treatment of the underlying arrhythmia with medication.
8 Μαΐ 2023 · Procainamide is a medication used to manage and treat ventricular arrhythmias, supraventricular arrhythmias, atrial flutter, atrial fibrillation, AV nodal re-entrant tachycardia, and Wolf-Parkinson-White syndrome. It is a Class 1A antiarrhythmic agent.
In 10 patients (29%), a catheter ablation was performed after treatment initiation. In 3 patients (9%), 2 catheter ablations were performed, 1 before and the other after procainamide initiation. Overall, 26 out of 34 (77%) patients underwent catheter ablation.
The recommended dose is 5 mg slow IV (over 5 minutes); wait 10 minutes, then if the first dose was well tolerated, give a second dose of 5 mg slow IV (over 5 minutes). An oral regimen is then initiated at 50 mg every 12 hours.
Adult Dosage for Procainamide: The use of procainamide is limited in ACLS for cardiac arrest, due to its requirement of slow infusion and its somewhat unknown effectiveness. If used for recurrent ventricular fibrillation and pulseless ventricular tachycardia, 20 mg/min should be given via IV infusion to a total dose of 17 mg/kg.
