It is a general term for the presence of proteins, including albumin, globulin, Bence-Jones protein, and mucoprotein in the urine. Almost half of the protein lost in normal urine is derived from the distal tubule, known as Tamm-Horsfall glycoprotein (THG).
A persistent positive urine dipstick test for proteinuria should prompt sending a sample for laboratory quantifi-cation by the ratio of albumin or protein to creatinine (see box 1). A systematic review has shown that testing of a random urine sample by protein:creatinine ratio had a sensitivity >90% to rule out pathology but low specificity
