According to the ACC consensus on management of bleeding due to oral anticoagulants, standard dose 4F-PCC is recommended for patients who require warfarin reversal due to major bleeding or urgent surgery. 1 A lower, fixed dose of 1000 or 1500 U is an alternative dosing option.
