Tennessee's CHOICES program provides older adults (age 65 and older) & adults with physical disabilities (age 21 and older) who are eligible for needed long-term services and supports in the home/community setting or in a nursing facility.
Unlike traditional fee-for-service Medicaid, TennCare is an integrated, full-risk, managed care program. TennCare services are offered through managed care entities. Medical, behavioral and Long-Term Services and Supports are covered by “at-risk” Managed Care Organizations (MCOs).
Tennessee Medicaid may help cover the cost of in-home care for eligible seniors through two programs: The CHOICES in Long-Term Care program, which is available statewide, and the PACE program, which is only available in Hamilton County.
Can Medicaid Help You Pay for Home Care in Tennessee? Medicaid is available for disabled Tennesseans or those over the age of 65 who require long-term care. While you can use Medicaid to pay for medical treatments often associated with skilled nursing care, it doesn’t cover home care costs directly.

Medicare A and B cover 100% of qualified home health services, with no out-of-pocket co-pays and/or deductibles. Medicare HMOs, Medicaid and private insurance cover many home health services. However, coverage varies and may require patient co-pays and/or deductibles.
