A risk matrix displays the basic properties, “consequence” and “likelihood”, of an adverse event and the aggregate notion of risk by means of a graph. It uses discrete categories of consequence, likelihood, and risk.
More specifically, this chapter focuses on the development and usage of a risk assessment methodology called Hazard Matrix (HM) and its application in Health, Safety and Environmental Management (HSE). The HM is a prioritization methodology suitable to be used in the analysis phase of a risk management program.
Assess and score the risk (i.e. probability X severity) using the Risk Matrix as per TPL Risk Assessment Matrix (RAM) (refer to Section 6.3). List out present controls (preventive and recovery). Assess the risk based on present controls. Reassess the medium and high risk to bring it down to acceptable risk.

Figure 1. Risk Assessment Matrix (RAM). The RAC quantifies the risk level associated with the hazard’s probability and severity ratings. Risk can fall into one of four levels. Each of the four levels requires specific actions to mitigate. The RAC helps to prioritize hazards such that those that pose the greatest risk can be addressed first.
The two variable risk matrix assesses the likelihood and consequence of a hazard. This assessment determines the level of risk associated with the hazard. Likelihood is the probability that something might happen. Consequence is defined as the most probable result of the potential incident. NOTE: Under some circumstances an alternative risk matrix
Another key step in the safety risk assessment process is safety assessment documentation and the use of Hazard Logs to document the output of hazards identification is also described and an example hazard log template provided. Some examples of hazards and information sources that could be used to identify hazards are also provided as annexes.
