This nursing care plan is for patients who are experiencing acute pain. According to Nanda the definition for acute pain is the state in which an individual experiences and reports the presence of severe discomfort or an uncomfortable sensation lasting from 1 second to less than 6 months.
Assess pain routinely with vital signs and perform a follow-up assessment after treatment. Pain is often considered the fifth vital sign. Frequent assessment is necessary at designated times and as needed. Pain can change after procedures, activity, or other treatments such as wound dressing changes.
Acute pain is a common nursing diagnosis that can be made based on a patient’s reported symptoms and physical examination findings. The nursing diagnosis for acute pain is “acute pain related to tissue damage, inflammation, or injury as evidenced by patient reports of pain and grimacing behaviors.”
