9 Φεβ 2022 · About this document and download. Notice: The Paycheck Protection Program (PPP) ended on May 31, 2021. Existing borrowers may be eligible for PPP loan forgiveness. A borrower may use this form only if the borrower received a PPP loan of $150,000 or less. Download .pdf.
You (the Borrower) can apply for forgiveness of your First or Second Draw Paycheck Protection Program (PPP) Loan using this SBA Form 3508S only if the loan amount you received from your Lender was $150,000 or less for an individual First or Second Draw PPP Loan.
The Direct Forgiveness Portal allows PPP borrowers to apply for forgiveness directly to the SBA using forms 3508, 3508EZ, or 3508S. Using MS Edge or Google Chrome browsers is recommended for an optimal experience. New Registration will be required to utilize this portal.
30 Ιουλ 2021 · If you are not eligible to use this form, you must apply for forgiveness of your PPP loan using SBA Form 3508 or 3508EZ (or lender’s equivalent form). Each PPP loan must use a separate loan forgiveness application form.

To apply for forgiveness of your Paycheck Protection Program (PPP) loan, you (the Borrower) may use the Form 3508EZ application or complete this application as directed in these instructions, and submit it to your Lender (or the Lender that is servicing your loan). Borrowers may also complete their application electronically through their Lender.
