Failure of this procedure leads to bipartite sesamoid bones 4. The most frequent mechanism of injury is a violent hyperextension of the pollex 2, 6, 7. A fracture may be caused also by a violent abduction of the thumb 6 and less frequently from direct trauma 3. In our case the injury was the result of a violent abduction of the pollex.

1 Μαΐ 2001 · Radiographic evidence of subsesamoid osteoarthritis was present in 23 cases. Patients were treated with excision of only the symptomatic sesamoid through a transverse palmar incision. The radial sesamoid alone was excised in 31 cases, the ulnar sesamoid in two cases, and both bones in three cases.
An open fracture of a thumb sesamoid associated with laceration of the palmar plate in a child was treated by reapproximating the palmar plate and the fracture fragments with sutures.
Recovery from sesamoiditis or a sesamoid fracture depends on the severity of the problem as well as the symptoms. Recovery can take a long time, with multiple weeks or months of wearing a cast and not putting any weight on the foot.
Case: Fracture of a sesamoid bone of the thumb is rare. We report a case of such a fracture after hyperextension of the thumb in a 26-year-old woman. After a volar plate injury was excluded, she was treated nonoperatively with 5 weeks of immobilization followed by physical therapy.
Two additional closed fractures of the thumb sesamoid were treated by splinting the metacarpophalangeal joint in comfortable flexion for 2 to 3 weeks. Normal hand function was restored in all the three patients.
