14 Οκτ 2005 · Antacids containing sodium bicarbonate should be avoided during pregnancy because they cause maternal or fetal metabolic alkalosis and fluid overload. Antacids should be taken at a different time than supplemental iron, because normal gastric acid facilitates the absorption of iron.
Sodium bicarbonate is a systemic alkalizer, which increases plasma bicarbonate, buffers excess hydrogen ion concentration, and raises blood pH, thereby reversing the clinical manifestations of acidosis.
The patient subsequently restarted her oral sodium bicarbonate tablets with initial improvement in her acidosis. However, despite ongoing oral supplementation she became more acidotic with bicarb of 14 and required IV repletion.
