Locate insurance company information, verify names in policies, and contact your state insurance department using the NAIC's lookup tool.
Instructions to search for an insurance licensee or to purchase a list of licensees. Go to the NAIC State Based Systems website (link below). Select Arizona as the Jurisdiction. Select Generate a Report. At Search Type, select Licensee. At Entity Type, select Individual or Entity. Complete the page using the criteria for your requested report.
File Form L193 to use any name other than applicant's genuine name when engaging in business under an Arizona insurance license. Visit the producers page of the Arizona Department of Insurance website for Form L193 and additional information about this requirement.
Rules and Requirements. Submit initial license for individuals electronically. Add lines of authority (amend) to an existing active license. Reinstate/reapply through NIPR's Non-Resident License Application. Applicants must apply for the License Classes and Lines of Authorities they qualify for.
