If you have more than three family members, photocopy this page before you start completing it or print it from our Web site at www.cic.gc.ca. Make sure you have enough copies to fill in details about all your family members.

APPLICATION FOR VISITOR VISA (TEMPORARY RESIDENT VISA) If you need more space for any section, print out an additional page containing the appropriate section, complete and submit it with your application. If you are submitting an online application, you do not need to print and sign the form.
