Low-dose aspirin is a widely used agent for preventing cardiovascular disease, and various guidelines also recommend its use in subjects with diabetes. Several primary prevention aspirin trials- prior to 2000 suggested a reduction in myocardial infarction and stroke, although not mortality but at the cost of increased bleeding events.
Aspirin is the cornerstone of treatment in patients with cardiovascular disease, irrespective of diabetes status, being able to confer a 19% relative risk reduction per year in serious vascular events compared with placebo at long-term follow-up (6.7% vs 8.2% per year, p < 0.0001).
