1 Απρ 2021 · Under COBRA, group health plans must provide covered employees and their families with certain notices explaining their COBRA rights. Section 9501 of the ARP provides for COBRA premium assistance to help Assistance Eligible Individuals (as defined below in Q3) continue their health benefits.
Once your agency notifies ERS of a change in your employment status, ERS mails you a COBRA notification letter, the COBRA Election Form and a rate sheet, and instructions for paying your premiums. These are sent to the address ERS has for you on file.
If you have questions regarding your election for COBRA coverage, contact Blue Cross and Blue Shield of Texas toll-free at (800) 521-2227. If you have additional questions regarding your COBRA rights, you may contact the Texas Department of Insurance toll-free at (800) 252-3439.
6 Οκτ 2021 · ERS will provide you with a COBRA Election Form and COBRA Notification following the termination of your coverage. You and/or your dependents must formally elect continuation coverage on the form provided and submit the appropriate premium payment within 105 days of the date coverage terminated or the date of notice, whichever is later.
