By phone, simply contact The Standard’s Claim Intake Service Center at 800.378.2395 Monday through Friday between 6:00 a.m. and 6: 00 p.m. Mountain Time. By paper, download the LTD Claim Packet. Be prepared to provide the following information: Employer: Albuquerque Public Schools. Group Policy Number: 645746. Your name and social security number
