The ideal staged decannulation protocol should include a reduction in the tracheostomy tube (TT) size, endoscopic evaluation of the airway, a trial of daytime capping of the TT at home, a capped sleep study, and admission for decannulation and observation for 24–48 h. In the majority of children, the tracheostoma will heal spontaneously.
