  2. CampusCare works in conjunction with the Student Health Service Fee to provide student at UIC comprehensive health benefits. Information regarding the benefits provided by CampusCare can be found here .

  3. Benefits. All services must be medically necessary, authorized by a CampusCare Primary Care Provider (PCP) and provided at a Contracted Network Provider unless they are authorized by the CampusCare Medical Director in order to be covered under the rates listed below.

  4. Waiver & Reinstatement Forms. Students are automatically enrolled in CampusCare and assessed the CampusCare fee on their tuition account along with mandatory fees. Students have the option of waiving CampusCare by showing proof of current insurance prior to the deadline date for the semester.

  5. All covered services must be provided or authorized by a CampusCare Health Center Provider, unless they meet the Medical Emergency guidelines as stated in the Certificate of Coverage or obtain a preauthorized referral by the CampusCare Medical Director or designee.

  6. CampusCare is the student health insurance program. All students must maintain health insurance throughout their student careers. This can be via CampusCare or other insurance coverage. If students have alternative insurance coverage, they must submit an online waiver for the CampusCare program.

  7. All UIC students are required to have health insurance. Registered students are automatically enrolled in CampusCare health insurance through UIC. See the CampusCare webpage for details about cost and coverage.
