Waiver & Reinstatement Forms. Students are automatically enrolled in CampusCare and assessed the CampusCare fee on their tuition account along with mandatory fees. Students have the option of waiving CampusCare by showing proof of current insurance prior to the deadline date for the semester.
CampusCare is the self-funded student health benefit program that provides comprehensive health care benefits to eligible enrolled students and their covered dependents at the University of Illinois Chicago, Rockford and Peoria campuses.
For waiver, enrollment, eligibility, general benefit questions or plan issues contact University Health Plans at 833-251-1718. For specific benefit or claims questions contact Aetna Student Health at 800-841-3140.
If you do not wish to remain enrolled in CampusCare, you may waive out of the program by providing proof of your private insurance coverage that meets UIC and State of Illinois requirements. Please visit the CampusCare website for details on waiver deadlines and requirements .

Dates & Fees. Coverage under CampusCare begins and ends on the following dates for the current Academic Year. Students who wish to waive coverage, reinstate coverage, or enroll/drop eligible dependents must complete and submit the appropriate online form (s) by the deadline dates posted below:
10 Αυγ 2024 · Waiver Process. Students covered by a comprehensive health insurance plan that meets the waiver requirements can opt out of the SHIP by completing the annual waiver form by the deadline of September 1, 2024. Before waiving, compare the cost, coverage, and benefits of the SHIP to your existing plan.
