Abdominal ascites is most commonly caused by portal hypertension from liver cirrhosis. When present, portal hypertension is associated with an elevated serum-ascites albumin gradient (SAAG) ≥1.1 g/dL.
A common medical cause of presentation with a distended abdomen accompanied by dull percussion (with or without shifting), fluid thrill, and bulging flanks is ascites. Further imaging modalities such as ultrasound or CT scan of the abdomen and abdominal paracentesis can be used to evaluate underlying etiology of abdominal distension.
