NVIC 05-17 (Guidance for reporting and investigation where the US is Substantially Interested State) Navigation and Vessel Inspection Circular 01-17. As of July 1, 2016 new 2692 forms and addendums are to be used for reports of marine casualties.
This form satisfies the requirements for written reports of accidents found in the Code of Federal Regulations for vessels, Outer Continental Shelf (OCS) facilities, mobile offshore drilling units (MODUs), and diving. The kinds of accidents that must be reported are described in the following instructions.

Navigation and Vessel Inspection Circular (NVIC) 01-15 lays out the requirements for reportable incidents in accordance with 46 CFR Part 4. There are very specific reports – both immediately (generally by voice) and then written within 5 days.
