Navigation and Vessel Inspection Circular 01-17. As of July 1, 2016 new 2692 forms and addendums are to be used for reports of marine casualties.
This form satisfies the requirements for written reports of accidents found in the Code of Federal Regulations for vessels, Outer Continental Shelf (OCS) facilities, mobile offshore drilling units (MODUs), and diving. The kinds of accidents that must be reported are described in the following instructions.

Use these forms to document oil spill incidents and expenditures. The reports shall be directed to either the nearest Captain of the Port or to the National Response Center (NRC) toll free number 1-800-424-8802. Spill reports must be made to the Coast Guard (within U.S. waters).
How to Save and Use the Word Template Form: The 209 template (.dot file) can be edited to match most incident situations and can be saved into the Word template directory.
According the 46 CFR 4.05-10(a) the owner, agent, master, operator, or person in charge, shall within five days, file a written report of any marine casualty required to be reported under 4.05-1. The written report must be submitted on Form CG-2692 (Report of Marine Accident Injury, or Death).
