Valproic acid and its derivatives, hereafter given the generic term 'valproate', are weak blockers of sodium ion channels that produce a weak inhibitor of enzymes that deactivate gamma‐aminobutyric acid (GABA) (such as GABA transaminase) (Fawcett 1989).

The aim of this review is to analyze both clinical and preclinical aspects regarding the use of valproic acid (N-dipropylacetic acid or VPA), moving from preclinical evidences of anticonvulsant activity and neuroprotection to clinical data on efficacy and tolerability profile both in the pediatric and adult population.

Its pharmacological effects involve a variety of mechanisms, including increased gamma-aminobutyric acid (GABA)-ergic transmission, reduced release and/or effects of excitatory amino acids, blockade of voltage-gated sodium channels and modulation of dopaminergic and serotoninergic transmission.
