Providers should verify eligibility with Humana Military. The quickest, easiest way to do this is through provider self-service. Active Duty Service Members (ADSM), family members over age 14, retirees and family members will have one of two valid military ID cards shown below.

Verify the card bearer’s TRICARE eligibility online at www.tricare-west.com or by calling the Health Net Federal Services, LLC (HNFS) interactive voice response (IVR) system at 1-844-866-WEST (1-844-866-9378) .
