Records requests can be made online or by mail directly to the Vermont State Police. The Vermont Access to Public Records Act, Title 1, Chapter 5, Subchapter 3 of the Vermont Statues Annotated, provides for access to public records. The Act also includes specific exemptions from public inspection. Those exemptions are set forth in 1 V.S.A. § 317.
To request a police report or other public record by mail, you will need: Record Request Form. VSP case number. Date of the incident. Name of all parties involved. Type of incident. Check payable to the VT Dept. of Public Safety. Mail your request to: Vermont State Police. Attn: Record Request. 45 State Drive. Waterbury, VT 05671-1300.
Foreign travel/Immigration. Processing of record check requests received via US Mail take five to seven (5 – 7) business days. All incomplete forms will be returned unprocessed. By coming into our office you can obtain your notarized record check in approximately 10 minutes.
This service allows users to purchase police reports and public records that are on file with the Department of Public Safety. These requests cost $20.00 for a report, $20.00 for a disc of photos, $45.00 for a video (either DUI or non-DUI incidents), and $45.00 for an audio recording.

To request public records from the Department of Public Safety, you must provide either the Vermont State Police Case Number of the incident or the date of the incident along with the names of all parties involved and the type of incident.
