22 Αυγ 2024 · Healthy Indiana Plan applications can be made online at HIP.IN.gov, by mail, fax or phone, or by visiting a local FSSA Division of Family Resources (DFR) office. You can find your local DFR office by going to HIP.IN.gov .
How do I apply? Applications are available online or by mail, or can be picked up at any Division of Family Resources office. 30 Οκτ 2023 · The Healthy Indiana Plan (HIP) is an affordable health plan for low-income adult Hoosiers between the ages of 19 and 64. It is sponsored by the state and for some members requires a small monthly payment through your Personal Wellness and Responsibility (POWER) Account.
To qualify for the Healthy Indiana Plan, applicants must be between the ages of 19–64 and meet the following eligibility requirements: Hoosiers with incomes in 2020 up to $17,829 annually for an individual, $24,078 for a couple or $36,590 for a family of four are generally eligible to participate in HIP.
