Use this form to notify the Department of Health Services (DHS) of asbestos abatement activities as required under Wis. Admin. Code ch. DHS 159. Do not use this form to notify the Department of Natural Resources (DNR). See page 2 for instructions. Notify online at https://health.wisconsin.gov/Waldo/Notify.
Notification to the Department of Natural Resources (DNR) or the Department of Health Services (DHS) may be submitted using Form 4500-113, online system (http://dnr.wi.gov/topic/demo/asbestos.html) or this paper version.

Enter your company's DHS number. It is printed on its asbestos company certificate. Only certified companies may notify asbestos projects. Please call 608-261-6876 with any questions about this requirement. To apply for company certification, visit https://www.dhs.wisconsin.gov/waldo/Index.htm.
A Notification for Demolition and/or Renovation (Form 4500-113), can be submitted using the online Asbestos Renovation and Demolition Notification System (ARDN). To access the system, you must register on the DNR Switchboard .
