A long form record includes the full death registration including the medical conditions reported and other circumstances which led to death. PROCESSING TIMES FOR MAILED REQUESTS
PART I. Enter the chain of events - diseases, injuries, or complications - that directly caused the death. DO NOT enter terminal events such as cardiac arrest, respiratory arrest, or ventricular fibrillation without showing the etiology.

Certified copies of death certificates OUTSIDE OF THE CITY OF DETROIT may be obtained in person or by mail. Please print out the appropriate application and visit us, or forward a written request (along with a copy of Driver's License or State I.D.) to the address below.
Order certified copies of death records online. At any of the Ottawa County Clerk locations (see right-side column). Fee is $15; additional copies of the same death certificate requested the same day are $6 each. Cash, check, money order or Visa, MasterCard or Discover are acceptable as payment. application for certified death record (PDF).
Anyone is eligible to receive a certified copy of a death certificate (Michigan Compiled Law 333.2882 (1)). The death must have occurred in Genesee County. The fee for a certified copy is $25.00.
