  5. I AGREE THAT ALL OF MY QUESTIONS HAVE BEEN ANSERED TO MY SATISFACTION. I CONSENT FOR MYSELF OR MY MINOR CHILD TO RECEIVE COUNSELING SERVICES WITH Jean Dixon LPC (DBA Jean T. Dixon, LPC). I CERTIFY THAT I HAVE THE LEGAL RIGHT TO SEEK AND AUTHORIZE TREATMENT FOR MYSELF AND/OR MY MINOR CHILD.

  6. Informed consent provides an opportunity for counselors to clearly explain the treatment plan, including goals, duration, and any potential challenges or requirements. It allows clients to have a realistic understanding of what to expect from the counseling process.

  7. An effective informed consent form will: Create trust between the therapist and the client. Reduce risk for both the therapist and the client. Increase the client’s understanding of the proposed treatment or therapy. Promote ethical practices by respecting the client’s right to dignity, autonomy, and integrity.
