Certification forms. The FMLA does not require the use of any specific certification form. The Department has developed optional forms that can be used for leave for an employee’s own serious health condition (WH-380-E) or to care for a family member’s serious health condition (WH-380-F).
An employer may require an employee seeking FMLA leave due to a serious health condition (their own or a family member’s) to submit a medical certification to verify the employee’s need for time off. The employer may not request a certification for leave to bond with a newborn child or a child placed for adoption or foster care.
This form is for employees who need to return to work after FMLA leave. It requires health care provider to verify fitness for duty, any restrictions and duration, and to avoid providing genetic information.

An employee on a medical leave under the Family and Medical Leave Act (FMLA) must present this Fitness for Duty Certification to their supervisor prior to returning to work. The Family and Medical Leave Act (FMLA) guidelines are applied to employees who are on paid or unpaid leave.
