His rank is Master Chief Petty Officer, the highest enlisted rank in the naval system, equivalent to Sergeant-Major in the army system. In theory, any officer down to the freshest Ensign could give the Chief an order, but in practice, they’d know to defer to his experience.

Yes, Master Chief Petty Officer is indeed a rank (in-game and real world). Which is why people call John "Master Chief", because they're addressing his rank. Arbiter is also technically a "rank/position", which is why the Halo Wars Arbiter (whom is a different elite from the Halo 1-5 Arbiter) is also called Arbiter, they are addressing his rank.
