Sleep Diary. Track your habits and get on the path to a better night’s sleep! Sleep is a basic need, as important to our health and well-being as air, food and water. When we sleep well, we wake up feeling refreshed, alert and ready for daily activities.
List all medications, vitamins, and supplements you took today. Approximately 2-3 hours before getting to bed, I consumed: Alcohol. 1+ glasses of water, juice, milk, etc.
TWO WEEK SLEEP DIARY. INSTRUCTIONS: (1) Write the date, day of the week, and type of day: Work, School, Day Off, or Vacation. Put “M” when you take any medicine. Put “A” when you drink alcohol. Put “E” when you exercise. (2) Put the letter “C” in the box when you have coffee, cola or tea.
